I know there is the ASIO plugin as an alternative, but many soundcards are limited by the drivers when using ASIO (no 24bit and/or 96kHz support) and in many cases (e.g. with ASUS cards) ASIO doesn't produce the highest quality sound output (according to various RMAA tests). Kernel Streaming is the only option for bit-perfect sound with minimal distortion.
Using Kernel Streaming under Windows 7 forces exclusive mode output and disables the Windows master volume slider (output is fixed at 100%).
There's also the WASAPI input plugin if you're using Windows Vista/7, but it doesn't work with cards that always request multichannel (6.1/7.1) output instead of stereo.

No, there's none. You could give
ASIO2KS
a go, combined with xmp-asio.
