Save single MIDI file using 2MIDI

Using 2MIDI, I converted a .mod file into

test.mid
playmid.cfg
13 .wav files of the form test0##.wav

playmid.exe plays this output set very well.  But is there a way to save all this as one single MIDI file?  test.mid alone does not play very well in Media Player or Winamp.

If this is not possible, then is there a way to associate the .wav files with test.mid in Media Player?

No, I'm afraid you can't embed the samples in the MIDI file. If you're a registered user, you can export the samples to a SF2 or DLS file. The Winamp MIDI plugin (using DirectMusic), for example, allows you to choose a custom DLS file. Here's an example conversion...
